OFFICIAL RULES FOR THE 2023 FILTEC CUSTOMER SURVEY PRIZE DRAWING
Contest start: January 26, 2023
NO PURCHASE IS NECESSARY TO ENTER OR WIN. APPLICABLE ONLY IN THE CONTINENTAL UNITED STATES. ENTRY INTO THIS PRIZE DRAWING CONSTITUTES ACCEPTANCE OF THESE PRIZE DRAWING RULES. VOID WHERE PROHIBITED.
1. ELIGIBILITY
To be eligible to enter the 2023 Filtec Customer Survey Prize Drawing (hereinafter, “Prize Drawing”), you (i) must be a legal U.S. resident who is at least 18 years of age, or above the age of majority in your state of residence, at the time of the launch date of the Prize Drawing, and you (ii) must have received an invitation from the Sponsor, FILTEC, also known as INDUSTRIAL DYNAMICS (hereinafter, “The Company”), to complete the Survey. Employees, officers, directors and agents of The Company, their subsidiaries and affiliated companies and their immediate families and those living in their households, are not eligible to participate in the Prize Drawing. Entry requires the exchange of information at the completion of the Survey to verify it is a serious submission. The Company reserves the right to exclude any entries from the drawing based on the validity of the entry. Void where prohibited. All federal, state and local laws and regulations apply.
2. TERM
The Prize Drawing will begin on January 26, 2023 at midnight Pacific Standard Time (PST) and will end at 11:59 PT on March 1, 2023 (“Term”).
3. SPONSOR
Filtec located at 3100 Fujita Street, Torrance, California, USA 90505 is the sponsor of the Prize Drawing (“Sponsor”).
4. TO ENTER
NO PURCHASE IS NECESSARY TO ENTER OR WIN. Each Prize Drawing is open to those eligible persons who complete the Survey and who submit their contact information to the Sponsor. To enter, eligible persons must complete the Customer Survey. Surveys are to be completed online through the following web site, or similar website designated by Sponsor:

Upon completion and submission of a survey, you will automatically be entered.
LIMIT: ONE (1) ENTRY PER COMPLETED SURVEY. You may only complete one survey and you will not be entered more than once. Subsequent entries will be deleted. All entries become the property of the Sponsor. Sponsor is not responsible for late, misdirected, incomplete, or for any computer-related, online, telephonic, or technical malfunctions that may occur. Entries are void if they are incomplete, damaged, irregular, altered, counterfeit, produced in error, or obtained through fraud or theft. By entering, you agree to be bound by these Official Rules.
Personal information (if any) collected through responses to the Claims Satisfaction Survey will be governed by Sponsor’s privacy policy.
5. WINNER
Prize: Ten (10) prize winners will be randomly selected for each Prize Drawing to receive ONE (1) $50 Gift Card each.
All decisions on all matters pertaining to the Prize Drawing are final. Sponsor reserves the right to substitute a prize of equal or greater value. Odds of winning depend on the number of eligible entries received.
6. WINNER NOTIFICATION
Random selection of the potential winners will occur during the week following the close of the survey on March 1, 2023. Prize drawing will be conducted at The Company, at 3100 Fujita Street, Torrance, California, USA 90505. The prize winners will be notified by e-mail or regular mail, at Sponsor’s discretion, within thirty (30) days from the date of the winner selection. Prize winners will have seven (7) days from notification to claim their prize. Sponsor and its affiliates, subsidiaries, employees, agents, officers and directors, advertising and promotion agencies, and their respective officers, directors, employees and their respective agents will not be liable for unsuccessful efforts to notify a winner. The prize will be delivered by U.S. First Class Mail within the United States only. No prize transfer, assignment or substitution by winner permitted. Sponsor reserves the right, in its sole discretion, to substitute the prize for a prize of equal or greater value should all or part of a prize become unavailable for any reason. If the prize winner declines the prize, fails to claim the prize, is unavailable for prize fulfillment, fails to abide by the Official Rules, or is ineligible, Sponsor may select an alternate winner from all remaining eligible entries.
Where lawful, the winner may be required to sign an Affidavit of Eligibility, Release of Liability and/or a Publicity Release. Failure to return the required documents within the time period specified thereon may result in forfeiture of prize and random selection of alternate winner from remaining eligible entries. Except where prohibited by law, by accepting the prize, winner consents to Sponsor’s use of his/her Survey responses to be used, anonymously, in advertising and publicity purposes in any and all media worldwide (including winners list) without additional compensation or notification.
7. LIMITATION OF LIABILITY
(a) Sponsor reserves the right at its sole discretion to cancel, terminate, modify or suspend any or all of the Prize Drawings.
(b) By participating, entrants and winners release Sponsor, its affiliates, parent, and subsidiary companies, its employees, agents and officers, advertising and promotion agencies, as well as all others associated with the development and execution of this Prize Drawing, from and against any and all liability with respect to or in any way arising from the Prize Drawing and the awarding and use of the prize. Sponsor assumes no responsibility for any error, omission, interruption, deletion, defect, or delay in operation or transmission; communications line failure; theft or destruction of or unauthorized access to Prize Drawing entries or entry forms; or alteration of entries or entry forms. Sponsor reserves the right to correct clerical or typographical errors in promotional materials or any materials related to the Prize Drawings.
(c) Participants also agree (a) to be bound by these Official Rules; and (b) that the decisions of the Sponsor are final on all matters relating to the Prize Drawing. The Winner agrees to be bound by these rules and the decisions of Sponsor, whose decisions are final and binding.
(d) Before being declared a winner, each selected entrant will be required to sign a confirmation and acceptance confirming compliance with the Prize Drawing Official Rules and acceptance of the prize as offered the confirmation and acceptance must be returned within seven (7) business days of the notification date indicated on the documents, unless otherwise stated, or the selected entrant will be disqualified, and the prize forfeited.
8. LAWS
This Prize Drawing is governed by the laws of the State of California, without regard to the conflicts of law provisions.
